COVID-19 Information Center

  • Mask Guidelines

    Masks are temporarily required for all residents, patients, staff, and visitors at Country Manor Health Care & Rehab Center.


    Signs will be posted throughout our buildings indicating current masking status. 


    Guidelines are subject to change, and we ask for your continued caution and respect for our vulnerable population. If you or anyone in your household is feeling ill, please reschedule your visit. As always, our top priority remains the health and safety of our residents and patients.

  • General Visitation Guidelines

    • Indoor visitation is allowed for all Health Care Center and Rapid Recovery & Aquatic Center communities.

    • Visitors must sign in.

    • Visits do not need to be scheduled.

    • There is no limit on number of visitors per day, frequency of visits, or length of visits.

    • There is no limit to the number of visitors that a resident can have at one time.

    • If the county COVID-19 community level of transmission is high, all residents and visitors regardless of vaccination status, should wear face coverings or mask and physical distance at all times.

    • While not recommended, residents on transmission-based precautions (quarantine) can still have visitors. These visits should occur in resident room and resident should wear a facemask. (Visitors will be educated on potential risk of visiting and precautions necessary in order to visit)

    • All visitors must be able to follow the “Core Principles of COVID-19 Infection Prevention” at all times. If visitors are unwilling to adhere to these safety principles, visitation rights may be restricted.

  • Indoor Visitation During Outbreak Investigation

    If a COVID-19 case is reported within the facility, we strongly encourage against visiting until facility-wide contact tracing and testing have been performed. This is the safest, most effective way to prevent the virus from spreading.


    If you choose to visit during this time, you will be educated on potential risks and you must wear face coverings or mask for the duration of the visit, regardless of vaccination status.

    We will return calls to the best of our ability. Our priority is caring for you loved ones. We appreciate your patience and understanding that we are being forced to triage calls at this time. Emergency situations must be dealt with first.


    IMPORTANT - We ask that a primary, designated family member place calls and disseminate information to other members of the family. This will provide more efficient and timely communication for all parties involved. Thank you in advance for your help with this process.
